Advance Care Planning
Advance Care Planning (ACP) is all about preparing for future medical care decisions. It is a process which involves discussing your own unique values, your goals for future care, and identifying who you would like to speak on your behalf . It truly is the difference between you being in control of what happens and someone else, who may not know you, making these decisions for you. The end result of these discussions are an Advance Directive, which includes a living will and a designation of a health surrogate, who will help makes decision if you can't.
We can help make this often daunting and confusing process much simplier by:

Advance Care Plans are free to fill out and available in many different formats. These conversations are also covered by Medicare during an annual welness visit. The services we provide are expert guidance, education, and assistance with ensuring these documents reflect your values, as well as more time than primary care physicians may be abl to spend.
